Question 1 of 4

1. Question

A patient has calcific AS, preserved LVEF, concentric LV remodeling, small LV cavity size, and reduced stroke volume. The mean transvalvular pressure gradient is lower than expected for the valve appearance.

What is the best physiologic explanation?

Preserved LVEF confirms normal forward flow
Correct Incorrect Correct answer

Reduced stroke volume can lower peak jet velocity and mean gradient despite significant valve obstruction
Correct Incorrect Correct answer

The gradient is independent of transvalvular flow
Correct Incorrect Correct answer

Concentric remodeling excludes clinically meaningful AS
Correct Incorrect Correct answer

Correct

Incorrect
Question 2 of 4

2. Question

A patient with moderate-appearing calcific aortic valve restriction has anemia and a high cardiac output state. Peak jet velocity and mean transvalvular pressure gradient are higher than expected for the valve appearance.

What is the best explanation?

High forward flow can increase velocity and gradient without a major change in valve morphology
Correct Incorrect Correct answer

High gradients always indicate severe anatomic valve obstruction
Correct Incorrect Correct answer

Anemia lowers transvalvular velocity
Correct Incorrect Correct answer

Loading conditions do not affect AS hemodynamics
Correct Incorrect Correct answer

Correct

Incorrect
Question 3 of 4

3. Question

An echocardiogram shows a small calculated aortic valve area, but peak jet velocity and mean transvalvular pressure gradient are lower than expected. The LVOT tracing appears suboptimal.

What is the best next step?

Conclude that the patient has mild AS
Correct Incorrect Correct answer

Ignore the valve area and use the gradient only
Correct Incorrect Correct answer

Review measurement quality and Doppler data before interpreting the valve–LV–flow pattern
Correct Incorrect Correct answer

Assume the discordance is physiologic and requires no further review
Correct Incorrect Correct answer

Correct

Incorrect
Question 4 of 4

4. Question

A patient with long-standing AS has concentric LV hypertrophy, preserved LVEF, enlarged left atrium, and Doppler findings suggesting elevated filling pressures.

What does this pattern suggest?

The LV is unaffected because LVEF is preserved
Correct Incorrect Correct answer

Chronic pressure overload has contributed to LV remodeling, reduced compliance, and higher filling pressures
Correct Incorrect Correct answer

Elevated filling pressures exclude AS-related physiology
Correct Incorrect Correct answer

Preserved LVEF rules out reduced ventricular reserve
Correct Incorrect Correct answer

Correct

Incorrect
